The Outreach Manager is responsible for driving growth within an assigned territory by developing and nurturing relationships with referral sources, healthcare partners, and community organizations. This role helps more individuals access home and community-based services by increasing awareness, identifying opportunities, and supporting a steady pipeline of new clients. Working closely with the State Director, VP of Strategy and cross-functional teams, the Outreach Manager plays a key role in regional growth and visibility.

Primary Duties and Responsibilities

Driving Growth through Referrals & Admissions (80%)

Own achievement of territory-level growth goals through execution of leading (e.g., outreach activities) and lagging (e.g., referrals, admissions) metrics.

Conduct regular outreach with partners to identify individuals who may benefit from our services through key sales activities — such as calls, emails, 1:1 meetings, and in-service presentations — to build awareness and drive engagement.

Effectively communicate the organization's value propositions to referral sources and partners to win referrals.

Build strong, trust-based relationships with referral sources including case managers, service coordinators, MCO care coordinators, community partners.

Represent the organization at community events, provider collaboratives, and partner meetings to elevate visibility and strengthen partnerships.

Gather and share field insights to help refine outreach strategies and improve responsiveness to local needs.

Focus on generating new opportunities in priority programs and populations through intentional, consistent field activity.

Activity Tracking, Reporting & Team Collaboration (15%)

Accurately log all outreach and engagement activities in the CRM (e.g., Salesforce) to ensure visibility into the growth pipeline.

Monitor outreach performance and adjust tactics as needed to support success.

Collaborate with intake and operations teams to ensure smooth handoffs and high-quality client onboarding.

Actively participate in team meetings, strategy sessions, and training.

Partner with internal stakeholders to align outreach efforts with program availability and operational readiness.

Qualifications and Education Requirements

1+ years of experience in outreach, community engagement, relationship management, or growth-related roles.

Experience in healthcare, home care, social services, or Medicaid-managed programs is strongly preferred.

Proven ability to build and maintain partnerships across diverse stakeholders and settings.

Comfortable in a field-based role with significant independent work and local travel.

Proficient in using CRM systems (preferably Salesforce) to track contacts and outcomes.

The base salary range for this position is $70,000 to $100,000 per year. The base pay actually offered may vary depending on the candidate's geographic region, job-related knowledge, skills, and experience among other factors. This is in addition to a variable compensation plan and full range of medical, financial, and/or other benefits, dependent on the level and position offered.
