As the Community Partnerships Manager, you'll be responsible for promoting our unique agency in the community, with a focus on building strong relationships with referral sources and other community partners. Reporting directly to the CEO, you will have the opportunity for growth and development in a rapidly expanding company.

Primary Duties:

  • Effectively generate leads and referrals from community and payer partners through in-field and virtual sales activity, with in-field preferred whenever possible
  • Spend 80% of working time within your designated territory conducting outreach on key targets (80% customer-facing)
  • Consistently schedule weekly meetings with new target contacts to educate new sources on available services
  • Conduct in-service and educational presentations to referral sources and community partners to drive service knowledge and value
  • Achieve or exceed minimum KPIs for both leading and lagging metrics 
  • Create strong commercial territory sales plans, with support of CEO and Strategy Team, to achieve territory sales goals
  • Effectively maintain all lead, contact, and activity information in Salesforce in a timely and responsible manner
  • Provide feedback on marketing materials and messaging to help guide targeting and outreach strategy
  • Participate in philanthropy events on behalf of the company
  • Act as subject matter expert to community partners to drive utilization of services for eligible members across key accounts in a territory
  • Pursue internal learning opportunities to develop sales and customer management skills beyond entry level 
  • Conduct initial qualification call with case manager / family to determine eligibility for care, and collaborate with intake team and other cross-functional departments to ensure smooth handoffs and high-quality client onboarding

Qualifications:

  • Bachelor’s degree in related field (preferred)
  • Ability to build and maintain relationships with a wide variety of individuals, both internally and externally
  • Effective and positive written and verbal communication skills
  • Proficiency in using CRM systems for tracking, organization, and planning
  • Planning and organizing: Sets goals, plans commercially, and pursues strategic actions to achieve them
  • Coachable: welcomes constructive feedback and ongoing professional development
  • Independent means of transportation, valid driver's license, and able to drive
