As the Outreach Coordinator, you will be responsible for promoting our unique company in the community, with a focus on building strong relationships with referral sources and other community partners. In this role, you will have the opportunity for ownership and growth in our steadily expanding company.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES

  • Identify?primary decision makers and build positive working relationships with members of the social services and medical community and partners.

  • Build awareness?of our company as the preferred home health care provider in our area, achieving and exceeding company goals for census growth.

  • Promote?the company through regular visits in a manner that reflects our quality of care and unique capabilities.

  • Implement, coordinate, document,?and report consistent sales activities with multiple contacts in each referral source to secure new referrals, build census, and meet established goals.

  • Conduct initial qualification inquiries with clients and their family members as needed to explore eligibility for care, and coordinate with an Intake team for further support.

  • Accountable for all responsibilities in accordance with the job function including but not limited to:

  1. Flexing schedule to meet company needs, including evenings, weekends and holidays if necessary.

  2. Ensuring continual personal and professional development 

  3. Enhancing the agency’s reputation in the community

 

KEY SKILLS AND QUALIFICATIONS

  • Bachelor’s degree in social work, human services, human resources, business, marketing, or related field preferred. Additional work experience may be substituted for educational requirements.

  • Minimum two (2) years of business development or sales work experience with a proven record of success.

  • Professional verbal and written proficiency in English required. Conversational language proficiency in Hmong is highly preferred.

  • The ability to interact with a variety of individuals professionally and effectively: building and maintaining relationships is a core function of the job.

  • Ability to be creative and generate ideas as they relate to sales, community education and collaborative partnerships.

  • Ability to influence decisions with demonstrated knowledge of human services systems and services and an empathetic understanding of community members’ needs.

  • Must have access to a personal vehicle to use for work purposes; possess a valid driver's license and have a clean driving record; and pass a background study with the Minnesota Department of Human Services.
